Prescription Transfers
The Apothecary accepts most nationwide prescription drug insurance plans and the ¶¶Òõ¶ÌÊÓƵ Boulder .
All patients are required to provide prescription insurance information and current billing information at the time of service. Please note that prescription coverage information may be on a separate card from medical insurance, depending on the plan and provider.

Transfer your prescriptions to the Apothecary
We can transfer prescriptions from your home pharmacy to the Apothecary at ¶¶Òõ¶ÌÊÓƵ Boulder. Please allow 24-48 hours to transfer. If there are any issues, we will contact you.
Please complete a Prescription Transfer Request Form for each prescription you would like to have transferred. Email is not a secure method of communication. To send prescription information securely, please fax them to us at 303-492-4874. If faxing forms is not an option, forms can be emailed to apothecary@colorado.edu.
Note: Many providers will not continue prescribing controlled substances for more than a month or two without seeing you.
If you wish to transfer a prescription for a controlled substance (such as stimulant medications for ADHD), please consult with your current prescriber regarding their comfort or ability to continue prescribing this medication for you.
If they are willing, you may need to bring a paper prescription from your provider to the pharmacy or your provider may E-scribe a prescription directly to the pharmacy.
Otherwise, please consider getting set up with a local provider as soon as possible.
Transfer your Apothecary prescriptions to another pharmacy
To transfer your prescription from the Apothecary to another pharmacy, please contact the pharmacy you wish to transfer your prescription to and have them call the Apothecary at 303-492-8553.
